Intuit supports our employees’ access to comprehensive health care, including a broad range of reproductive health care services, such as testing and treatment for infertility, procedures to restore fertility, oral contraceptives, and termination of pregnancy.

Cigna Choice Fund with HSA

  • In-network: Plan pays 90% after you pay your deductible.
  • Out-of-network: Plan pays 70% of usual, customary, and reasonable (UCR) charges after you pay your deductible.
  • Includes artificial insemination, in vitro, GIFT, ZIFT, elective egg freezing (cryopreservation, storage, and thawing), and termination of pregnancy.
  • Fertility benefits limited to $30,000 lifetime maximum for medical and $10,000 for prescriptions (through CVS Caremark).
  • Prescription contraceptives are covered through the pharmacy benefits.
  • Certain medical services may be eligible for travel and lodging benefits if there is no in-network provider available within 60 miles of your home. For more information, please see details in our plan documents on the My Benefits website, or contact your medical plan.

Cigna Managed Network Plan

  • You pay $30 (Cigna Care Network specialist) or $40 (non-Cigna Care Network specialist) per visit to a physician's office, a $200 admission copay for inpatient services, and a $100 copay per visit for outpatient services.
  • Includes artificial insemination, in vitro, GIFT, ZIFT, elective egg freezing (cryopreservation, storage, and thawing), and termination of pregnancy.
  • Fertility benefits limited to $30,000 lifetime maximum for medical and $10,000 for prescriptions (through CVS Caremark).
  • Prescription contraceptives are covered through the pharmacy benefits.

UHC Network Plan

  • You pay $15 copay for a primary care physician or $30 copay for a specialist; limited to diagnosis and treatment of underlying condition only.
  • Includes artificial insemination, in vitro, GIFT, ZIFT, elective egg freezing (cryopreservation, storage, and thawing), and termination of pregnancy.
  • Fertility benefits limited to $30,000 lifetime maximum for medical and $10,000 for prescriptions (through CVS Caremark).
  • Through the Fertility Solutions program, you can work with a fertility nurse to get information about infertility causes and treatment options; find doctors, clinics, and facilities that are right for you; and navigate the health care system.
  • Prescription contraceptives are covered through the pharmacy benefits.
  • Certain medical services may be eligible for travel and lodging benefits if there is no in-network provider available within 50 miles of your home. For more information, please see details in our plan documents on the My Benefits website, or contact your medical plan.

Kaiser (California and Georgia)

  • You pay $20 copay per visit for outpatient services and $100 copay per admission for inpatient services; limitations apply.
