Intuit provides basic life and accident insurance benefits at no cost to you. You buy additional coverage for yourself, your spouse, and/or your children.

Eligibility

Regular employees working more than 20 hours per week are eligible for benefits as of your hire date.

Life insurance

The life insurance benefit is provided through Sun Life Financial. This benefit pays two times your annual earnings (rounded up to the next $1,000), up to a maximum benefit of $1 million.

Your dependent basic life coverage is $10,000 for your spouse and $5,000 per child.

Optional life insurance

You can purchase additional personal life insurance coverage in multiples of $10,000 up to a maximum of $500,000 (proof of insurability is required).

You can purchase additional life insurance coverage for your spouse in multiples of $10,000 up to a maximum of $250,000 (proof of insurability is required). Rates depend on your spouse’s age, gender, and whether or not they use tobacco.

You can purchase additional life insurance coverage for your children in units of $5,000 up to a maximum of $20,000 per child.

If you apply for optional life insurance within 31 days of your becoming eligible, evidence of insurability is not required by Sun Life for the first $30,000 of coverage (employee and spouse).

If evidence of insurability is required, you will receive an email with instructions on how to complete your health statement online. Sun Life will notify you and Human Resources if you’re approved or if further information is required. Once approved, the appropriate deductions based on the optional life form will be reflected on your pay stub.

AD&D insurance

Accidental death and dismemberment (AD&D) insurance is provided through Chubb Insurance Company. AD&D insurance covers two times your annual earnings (rounded to the next $1,000) up to a maximum of $1 million (depending on type of injury).

Beneficiaries

You must name one or more beneficiaries for your life and accident insurance. If you die while covered by the plan, the beneficiaries you name will receive payment from the plan. You are the beneficiary of accident (not resulting in your death) insurance benefits. Learn how to name or change beneficiaries.
